Aid & Attendance Benefits May Help Vets Pay for Assisted Living

At MorningStar of Rio Rancho, we proudly welcome vets into our community.  In fact, throughout MorningStar communities, we have vets from all five branches of military service.  Recognizing the courage and self-sacrifice they made to serve our country, we are committed to helping them get the services they now need to improve their quality of life. 

One of the ways we help is to direct prospective residents who are vets or surviving spouses to local resources that can determine if they qualify for the Aid & Attendance benefit.  This monthly benefit payment is provided by the U.S. Department of Veteran Affairs and is available to qualifying wartime vets or their survivors.

Requirements to qualify include financial need and a physician’s order for regular assistance with activities of daily living.  In addition, the vet must have received an honorable discharge and served for at least 90 days of active duty with at least one of those days served during a period of foreign war.  In order for a surviving spouse to qualify, he or she must have been married to the vet at the time of their death and never remarried.  

The reason we recommend going to accredited VA advisors for assistance in filing a claim for an Aid & Attendance benefit, is they are authorized by the VA to help claimants in the preparation, presentation and prosecution of claims for the benefit.  Once approved for the benefit, the monthly payment goes directly to the qualified applicant and can be used to pay for any kind of assistance, including assisted living. The National Care Planning Council estimates that up to 25% of seniors could potentially apply for Aid & Attendance.

Falls are the Leading Cause of Injuries for Seniors

We know that falls can be dangerous, and in fact, they are the leading cause of fatal and non-fatal injuries for older Americans.  The Centers for Disease Control and Prevention’s (CDC) statistics state that one in four Americans age 65 years and older fall each year.  They report that, “every 11 seconds, an older adult is treated in the emergency room for a fall; every 19 minutes, an older adult dies from a fall.”

However, there are several risk factors that can be addressed that may help prevent falls.  The National Institute of Health (NIH) explains that it is not only chronic health conditions that lead to falls but also safety hazards within or around a senior’s home.

The NIH cites muscle weakness, especially in the legs, along with a lack of flexibility and endurance as a significant risk factor that puts seniors at risk for a fall.  Another factor increasing risk is an unsteady gait when walking as well as poor balance, which can be contributed to chronic health problems and lack of exercise.  In addition, older adults need to be sure to wear safe footwear and should avoid backless shoes or slippers, high-heeled shoes or shoes with smooth soles.     

Furthermore, a condition known as postural hypotension (when blood pressure drops too quickly when getting up from a lying down or sitting position) can increase the risk of falling.  There are several chronic conditions such as Parkinson’s or diabetes as well as certain medications that can cause this to happen.  Also vision problems, sensory difficulties, confusion and medications can lead to dizziness and unsteadiness, and result in a fall. 

The NIH advises seniors to report any fall to their doctor as is may be a sign of a “new medical problem that needs attention, such as an infection or a cardiovascular disorder.”  It could also be a signal that your medication needs to be adjusted or changed.

Are You Caring for a Loved One with Dementia?

The Alzheimer’s Association states that the number of adults suffering from Alzheimer’s disease is increasing and there is now an estimated 5.8 million Americans with the disease.  They add that every 65 seconds someone comes down with the disease.  Among that number includes approximately 200,000 people who are under the age of 65 who have younger-onset Alzheimer's.   Experts also predict that as we continue to live longer, the number of those age 65 and older with Alzheimer’s will grow to 13.8 million by 2050.  Right now, it is the sixth leading cause of death in the United States, and currently, cannot be prevented or cured.

These sobering statistics mean that more families are finding themselves in the situation of having a family member with Alzheimer’s or another form of dementia.  For those who are family caregivers, the Alzheimer's Association found it can take an overwhelming toll on them both physically and emotionally.  Their studies show that close to 60 percent of Alzheimer’s and dementia caregivers “rate their stress level as high or very high.”  Approximately 40 percent of family caregivers suffer from depression and required healthcare services related to the emotional and physical toll of their caregiving.

Experts recommend that if you are family caregiver that you reach out for help from a variety of sources including relatives, friends, and local community organizations.  Caregiver support groups can also be helpful as you can connect with other who are dealing with the same problems.  Respite care and day care centers can also give you some relief from the responsibility and difficulties of caring for someone with Alzheimer’s.

Learn More About Alzheimer’s Disease

The shorter days of winter can be especially tough on individuals with Alzheimer’s disease who suffer from the condition known as sundowner’s syndrome says author Rick Phelps who is afflicted with the condition.  He explains that when we lose daylight and are faced with the often dreary, dark cloudy days of winter, those with sundowner’s syndrome can get further confused. 

The Mayo Clinic describes sundowner’s syndrome as “a state of confusion at the end of the day and into the night.”  They explain that it is not an actual disease but rather a symptom of Alzheimer's and typically peaks in the middle stage of the disease.

In fact, the Alzheimer’s Association says that, “As many as 20 percent of persons with Alzheimer's will experience increased confusion, anxiety and agitation beginning late in the day. Others may experience changes in their sleep schedule and restlessness during the night.”  Researchers do not understand the exact cause of sundowning but it appears to be triggered by a number of factors including fatigue or an overreaction to the day’s sensory stimulation. The inability to see well in the dark or hormonal imbalances that occur at night could be the cause of some of the anxiety people experience.  

Furthermore, since many people with the syndrome do not get a good night’s sleep that can trigger behavioral problems.  In addition to anxiety and confusion, the condition can also result in aggression, anger, depression, pacing, fear, restlessness, hallucinations, paranoia, wandering and violence. 

Experts offer some tips to better cope with sundowner’s syndrome such as keeping your home well-lit during the evening hours and following a strict schedule for meals and bedtimes.  They recommend that activities be scheduled in the morning or early afternoon to give the person plenty of time to unwind before evening.  Sleep disturbances should be discussed with the person’s health care provider.

The Impact of Social Connections on Brain Health

According to an article published by the Frontiers in Aging Neuroscience journal from the Ohio State University Wexner Medical Center, it reports socials connections may be key to preserving memory.  The study, led by Elizabeth Kirby, an assistant professor of behavioral neuroscience at the university, explains their findings influence "a body of research in humans and animals that supports the role of social connections in preserving the mind and improving quality of life.”   

The university’s research involved studying a group of mice 15 months to 18 months old, which is the period in a mouse’s life when it starts to have significant memory decay.  For three months, some mice lived in pairs while others were housed in groups of seven.  The researchers put the mice through a series of trials each day that tested their recognition and memory skills.  Consistently the mice housed in larger groups improved their performance on the tests, which researchers believe may suggest they “used their hippocampus, an area of the brain associated with memory.”  

The study is important for humans as our hippocampus declines with age as it does in mice and other animals, and helps to identify the impact of social connections on brain health.  Kirby says seniors should “think about your living situation as you age. If you have the privilege of choosing where you live, make the choice that lets you be socially engaged as long as possible."

Be Proactive with Your Heath Care Visits

Most of us have walked out of our doctor’s office and quickly thought of a question we wished we had asked the doctor.  We know that doctors are busy and our time with them is short, but the medical profession says it is important for seniors to proactively communicate with their health care providers and control the appointment.  

They recommend putting together a list of questions you want to ask before your next visit.  This can help to give you a better understanding of your medical conditions and improve your ability to self-manage them.  You should also bring a list of all of the prescription drugs, over-the-counter drugs, and vitamins and supplements you are currently taking.

General questions to ask include: 

  • Are my medications still appropriate to my condition and should the dosage be lowered or increased?  

  • How are my blood pressure, cholesterol and glucose levels?

If you are experiencing symptoms, make a list of all of them and include frequency and the amount of pain or discomfort associated with them.  If your doctor diagnoses a disease, ask them how do you spell it and if you will need any tests or medications to treat it? Too many seniors are reluctant to ask questions of their health care providers but it is important to become educated about your medical conditions, treatments and possible health risks.  In fact, most doctors say they wish their patients would be more proactive and ask questions during their visit as it lets them know you understand your health conditions.

Best Care Practices for Those Suffering from Dementia

Over the last several decades, there has been an increased awareness of Alzheimer’s and other dementia-related diseases.  Along with efforts to develop medications to treat or to cure the disease, there has also been a move to advance best care practices for those suffering from the disease. 

In a publication from the Alzheimer’s Association titled the “Dementia Care Practice Recommendations for Assisted Living Residences and Nursing Homes,” they explore ways that professional caregiving can be more effective.  Their first recommendation is to make sure a resident is receiving proper food and fluid consumption.  They note that “Inadequate consumption or inappropriate food and fluid choices can contribute directly to a decline in a resident’s health and well-being.” 

The next point they discuss is pain management.  They cite the fact that many times people with dementia have trouble communicating pain so it is often overlooked and not treated.  However, the problem with not managing pain appropriately is that it can “result in behavioral symptoms and lead to unnecessary use of psychotropic medications.”

Another important practice recommendation from the report is that assisted living communities provide social engagement and meaningful activities for memory care residents.  By engaging in activities, it helps residents to retain their functional abilities and thus provides a better quality of life.   These interactions should be designed to do with residents, not to or for them.

Early Signs of Alzheimer’s Disease

The Alzheimer’s Association’s website is loaded with the most up-to the-date information regarding all the various aspects of Alzheimer’s disease.   Part of their extensive information includes a check list of 10 early warning signs of the disease.  This is important to know as an early diagnosis allows someone an opportunity to seek treatment and to make plans for the future.  While everyone’s journey is different, they recommend if you or a loved one is exhibiting signs of memory loss, to consult with a doctor.

The first sign of Alzheimer’s as stated by the Association is memory loss that disrupts a person’s daily life. This is the most common sign of Alzheimer’s, especially in the early stages.  Examples include forgetting important dates or events as well as recently learned information.  

The second sign is experiencing difficulties in developing a plan or working with numbers.  It may affect everyday tasks such as balancing a checkbook or keeping track of monthly bills. 

Next they cite new problems that develop in completing familiar tasks around the home, at work or leisure.  The person experiences difficulties when driving to a familiar location, managing a budget or remembering the rules of a favorite game. 

Fourth on the Alzheimer’s Association’s list is signs of confusion regarding time and place.  Examples include losing track of dates and what time of the year it is as well as forgetting where they are or how they got there. 

Another sign of Alzheimer’s is problems understanding visual images and spatial relationships. This can mean difficulty reading, judging distances and determining color or contrast.  Further along in the disease, a person may not recognize their own reflection. 

The sixth sign on their list includes experiencing new problems with words when speaking or writing.  Another sign is misplacing things and not having the ability to retrace their steps.  People suffering from Alzheimer’s disease may put things in unusual places. 

The eighth sign includes decreased or poor judgment such as giving away large amounts of money to fraudulent causes or paying less attention to grooming habits.  The ninth sign stated by the Association is withdrawing from work or social activities.  Last on the list is a significant change in mood and personality.  People with the disease can become confused, suspicious, depressed, fearful or anxious.  Please see the Alzheimer’s Association’s website for the complete details on the early warning signs of the disease.

Driving Safety as You Age

Older drivers can still be good, safe drivers, but there are some things to consider both for your safety and the safety of others.  The Mayo Clinic recommends that seniors stay as active as possible in their day-to-day life which helps to maintain overall strength, flexibility, and range of motion.  Drivers need to be able to turn and look over their shoulder and move their head with ease. They must be able to keep a firm grip on the steering wheel and legs need to be strong enough to apply pressure to the gas pedal or brake.  

A senior’s peripheral vision can decrease with age or from certain medications and other eye problems may cause drivers to have trouble seeing in the dark or experience blurred vision.  Hearing problems can interfere with detecting sounds of sirens from emergency vehicles or approaching cars. Be vigilant about scheduling vision and hearing tests regularly as part of your wellness care and follow your doctor’s advice as to how often they should be done.  Many problems can be corrected if caught early enough.    

Remember that certain medications can cause drowsiness so make sure you know the side effects of your prescription drugs and do not drive if you are feeling sleepy.  It may be a good idea to limit trips in the car to daylight hours and when the weather is good. Keep in mind that a person’s reaction time slows with age as do their motor skills.  You may need to re-evaluate your driving every so few months, especially if you find yourself having incidences of accidents that almost happen.

Planning before you leave:

  • Plan to drive on streets you know.

  • Limit your trips to places that are easy to get to and close to home.

  • Take routes that let you avoid risky spots like ramps and left turns.

  • Add extra time for travel if driving conditions are bad.

  • Don't drive when you are stressed or tired.

While you are driving:

  • Always wear your seat belt.

  • Stay off the cell phone.

  • Avoid distractions such as listening to the radio or having conversations.

  • Leave a big space, at least two car lengths, between your car and the one in front of you. If you are driving at higher speeds or if the weather is bad, leave even more space between you and the next car.

  • Make sure there is enough space behind you. (Hint: if someone follows you too closely, slow down so that the person will pass you.)

  • Use your rear window defroster to keep the back window clear at all times.

  • Keep your headlights on at all times.
